Directions for requesting your transcript.
Please Download the attachment, print and complete the form (incomplete request forms will not be processed)
Following the completion of the form please scan it and email to firstname.lastname@example.org
- Requests should be made before a transcript is needed.
- Requests are processed within 10 business days. Processing DOES NOT include mail time.
- Transcripts ordered on a Friday after 4:30 p.m. will begin processing on the following Monday.
|All other copies||$10.00 per copy|
- You may not request a transcript by phone or fax.
- You may not request a transcript to be faxed.
- If picking up your Transcript in person make sure you have a photo ID with you.
- If you would like us to mail your transcript for you please provided an envelope addressed to your school with a stamp.
- Most universities want transcripts mailed directly to them. Ensure that you know their requirements before ordering your transcript.
- PTHC does not provide electronic transcript services.
- If transcripts are not picked up within 30 business days, another order must be placed.
- All PTHC debts must be paid before transcripts are released.
Attachment: If you have an attachment that needs to be mailed with your transcript:
- • Please email us your transcript request, first and then,
- • Scan and email the attachment to email@example.com
- • Please include your full name, date of birth and Start Date within all correspondence so we can match it.
